Job summary

Country Villa Wellness & Rehabilitation is seeking a licensed social worker to identify and address psychosocial, emotional, and psychological needs of residents in a skilled nursing setting. The role emphasizes ethical practice, regulatory compliance, and collaborative care planning.

Responsibilities include conducting psychosocial assessments, documenting in line with federal/state policies, and supporting residents’ self-determination and transition planning.

Qualifications

• Certified, licensed or registered in the state of practice, required.
• One year of supervised social work experience in a healthcare setting working directly with geriatric individuals.
• Minimum of 2 years of nursing experience in a Skilled Nursing Facility preferred.

Responsibilities

• Completes social history and psychosocial assessment for each resident that identifies social, emotional, and psychological needs; completed relevant parts of the minimum data set (MDS).
• Maintains accurate and timely documentation in compliance with regulations and center policies, including MDS, care plans and progress notes.
• Supports resident rights and informed choice; documents refusals and educates about alternatives.
• Participates in development of an interdisciplinary plan of care addressing psychosocial needs and goals.
• Provides therapeutic interventions to assist residents with transition and adjustment to long-term care.
• Coordinates discharge planning and referrals for home care services prior to return to the community.
• Works with the interdisciplinary team to promote resident well-being and protect rights; addresses abuse per law and licensure.
• Assists in educating residents, families and staff to improve quality of life and care within the facility.
