Job summary

Villa, a 100+ bed skilled nursing facility, seeks a Social Worker passionate about holistic resident care. The role focuses on psychosocial assessments, admission support, and addressing social, emotional, and psychological needs of residents in a healthcare setting.

You will participate in interdisciplinary care plan meetings, document progress notes, assist with housing transitions as needed, and uphold state and federal long-term care guidelines. Full-time position in Minnesota.

Qualifications

• Must hold a Bachelor’s Degree in Social Work or related field.
• Licensed Social Worker in Minnesota required.
• Healthcare experience preferred.
• Must meet state and/or federal Long-Term Care guidelines.
• Strong communication, documentation, assessment and social work methods.

Responsibilities

• Perform psychosocial assessments and support residents' social, emotional, and psychological needs.
• Attend interdisciplinary care plan meetings and document progress notes.
• Assist residents with housing transitions when necessary.

Job Description

Position includes being responsible for the completion of psychosocial assessments, adhering to facility admission process and providing for each residents social, emotional and psychological needs. Actively participate in interdisciplinarycare planmeetings. Document progress notes that relate to residents care plan. Assist the resident with alternative housing as necessary.This is a full-time position.

Qualifications

• Bachelor's Degree in Social Work or related field.
• licensed Social Worker in the State of MN
• health care experience preferred
• must meet and state and/or Federal requirements for the position
• basic understanding of computer technology
• must have ability to read, write and follow oral and written directions at a level necessary to accomplish the job
• must have knowledge of State and Federal Long-Term Care guidelines
• Must have demonstrated skills in communication, documentation, assessment and social work methods techniques.
• must maintain regular attendance
