Job summary

Center for Elders' Independence (CEI) is a PACE organization providing integrated health care for the elderly through an Interdisciplinary Team (IDT). The Social Worker supports psychosocial assessments, care planning, and communication among participants, families and CEI staff.

The role focuses on helping frail seniors live in their communities and requires engagement with healthcare partners in California.

Qualifications

• Master's degree in social work from an accredited school.
• One year experience with frail/elderly population preferred.
• Bilingual ability preferred (Spanish, Korean, Cantonese, Mandarin).
• Valid California driver’s license and CPR/First Aid.

Responsibilities

• Develops and implements psychosocial assessments and care plans.
• Facilitates communication between participant, family and CEI staff.
• Conducts home visits and participates in IDT meetings.
• Makes referrals to community resources and coordinates with social services.
