The Center for Elders’ Independence (CEI) is seeking a Clinic Registered Nurse to provide nursing services at the CEI clinic under the Clinic Supervisor. This role emphasizes care planning, interdisciplinary collaboration, and patient-centered care in a PACE program supporting elderly participants in the community.

Responsibilities include assessing participants, updating plans of care, delivering skilled nursing care, and educating families and caregivers.

Qualifikationen

• Graduate of an accredited school of professional nursing.
• “Current California Registered Nurse License” is required.
• Minimum of 1 year acute or ambulatory care experience, preferably with elderly.
• CPR certification must be on file.

Aufgaben

• Assess participants’ status in person, initially and as needed, and contribute to care plans.
• Monitor for acute changes and adjust the plan of care accordingly.
• Provide skilled nursing care in a timely manner.
• Educate participants, families, and caregivers on care needs and safety.
