Job summary

Center for Elders Independence in San Leandro, CA, is seeking a Social Worker to join the interdisciplinary team and support elderly participants in living in their communities.

You will conduct psychosocial assessments, develop and implement care plans, and coordinate communication among participants, families, and CEI staff.

The role offers an hourly wage of $37.97 to $56.95, with placement based on experience, skills, and market data, aligned with CEI compensation practices.

Qualifications

• Master's degree in social work from an accredited school of social work.
• One year experience working with a frail and/or elderly population preferred.
• Demonstrated ability to work in a multicultural and multidisciplinary team environment.
• Ability to work with minimal supervision.
• Excellent oral and verbal communication and meeting facilitation skills.
• Valid California driver's license and access to reliable transportation.
• Current CPR and First Aid.

Responsibilities

• Develops and implements comprehensive, integrated plans of care for participants.
• Acts as an IDT member and communicates participant changes to team members.
• Conducts psychosocial assessments and maintains case documentation.
• Makes referrals to community resources for financial management and benefits eligibility.
• Respects confidentiality in accordance with HIPAA principles.
• Provides home visits and travels to participant locations as needed.
• Participates in conferences and care planning meetings.
