Job summary

WelbeHealth seeks a Social Worker to manage a caseload of frail seniors, conducting psychosocial assessments and care planning in coordination with an interdisciplinary team. You will document interventions, arrange resources, and support transitions between care settings.

The role emphasizes collaboration with primary care and other IDT members to ensure high-quality, person-centered care while meeting policy requirements and promoting wellness among participants.

Qualifications

• Minimum of 1 year experience with frail/elderly population preferred.
• Experience coordinating and facilitating care conference meetings preferred.
• CPR and First Aid certification within 180 days of hire and maintained thereafter.

Responsibilities

• Conduct psychosocial assessments and participate in IDT care planning.
• Deliver and document social work interventions including counseling and case management.
• Coordinate care transitions with physicians and IDT members.
• Initiate care conferences and provide discharge planning when disenrolled.
