Job summary

NHC HealthCare Bluffton in Okatie, SC is seeking a dedicated Social Worker for a full-time role. The position requires a BS or MSW and knowledge of case management and geriatric funding resources.

You will advocate for residents and families and collaborate with a multidisciplinary team to support patient-centered care. Responsibilities include coordinating discharge planning, reporting to the Director of Social Work, and maintaining confidentiality while helping to develop individualized care

Qualifications

• Bachelor’s or Master’s degree in social work from an accredited program.
• Knowledge of the medical case management process and needs of geriatric patients.
• Familiarity with funding resources such as Medicare/Medicaid and private insurance.

Responsibilities

• Work with individual patients and families toward adjustment to center life and crises.
• Advocate for patients and families to express and resolve grievances.
• Coordinate discharge planning functions.
• Report to the Director of Social Work Services on current status and needs.
• Participate in developing patient care plans with other disciplines.
• Maintain confidentiality of professional information.
