Job summary

Sprenger Health Care is seeking a qualified Social Worker to support our Bluffton-area residents. The role focuses on building strong relationships with residents and families, coordinating community resources, and assisting with admissions and discharges.

The ideal candidate holds a Social Work credential and has experience in long-term care, with excellent communication and organizational skills to navigate complex care networks.

Qualifications

• Minimum 2 years of experience in long-term care.
• Licensed Social Worker or Bachelor's degree preferred.
• Excellent communication and organizational skills.

Responsibilities

• Develop one-to-one relationships with residents and families.
• Establish relationships with area hospice agencies.
• Act as a liaison with social, health and community agencies.
• Coordinate with Activity Department to meet socialization needs of the residents.
• Oversee coordination of referrals for ancillary services (dentist, eye doctor, podiatry, psychology, psychiatry, and audiology).
• Assist in admissions and discharge process as needed.

Qualifications:

2 years of experience in long term care, licensed social worker or bachelor's degree preferred, must possess excellent communication and organizational skills.
