Job summary

Sprenger Health Care of Bluffton is seeking a qualified social worker or experienced professional to support residents and their families in a long-term care setting. You will develop relationships, coordinate with hospice and ancillary services, and assist with admissions and discharges while ensuring compassionate, patient-centered care.

The role emphasizes communication, organization, and collaboration with social, health, and community partners.

Qualifications

• 2 years of experience in long term care.
• Licensed social worker or bachelor's degree preferred.
• Excellent communication and organizational skills.

Responsibilities

• Develop one-to-one relationships with residents and families.
• Establish relationships with area hospice agencies.
• Act as a liaison with social, health and community agencies.
• Coordinate with Activity Department to meet socialization needs of the residents.
• Oversee coordination of referrals for ancillary services (dentist, eye doctor, podiatry, psychology, psychiatry, and audiology).
• Assist in admissions and discharge process as needed.

Qualifications:

2 years of experience in long term care, licensed social worker or bachelor's degree preferred, must possess excellent communication and organizational skills.
