Qualifications

• Bachelor's degree from an accredited School of Social Work or related human services field.
• One year of supervised social work experience in a healthcare setting.
• Experience with geriatric, skilled nursing, long-term care, or rehab populations.

Responsibilities

• Conduct psychosocial assessments and develop individualized care plans.
• Coordinate with residents, families, and interdisciplinary teams to identify needs and goals.
• Provide guidance, counseling, advocacy, and support to residents and families.
• Connect residents with community resources and financial assistance programs.
• Coordinate discharge planning and transitions to next level of care.
• Advocate for residents' rights and person-centered care.
• Maintain documentation and ensure regulatory compliance.
• Participate in quality improvement and team meetings.
