Legacy Healthcare Management is seeking a Licensed Social Worker in Bingham Farms, MI. The role focuses on identifying residents' social, emotional, and psychological needs, developing and implementing plans to maximize their potential, and educating families about post-acute options.

The position involves discharge planning, coordinating with interdisciplinary teams, and ensuring accurate documentation in the Resident Care Process. Experience in healthcare social work is preferred.

Qualifications

• Bachelor's degree in social work or related human services field required.
• 1 year of supervised social work experience in a health care setting required.

Responsibilities

• Complete and document Social Work and Psychosocial assessments for all residents within 3 days of admission or readmission.
• Ensure 3877/78 forms are completed accurately on admission and annually.
• Schedule and notify IDT for 72-hour Resident Care Conferences and quarterly/annual meetings.
• Complete MDS sections and the appropriate CAAs.
• Develop and update Care Plans including Psychosocial, Discharge Planning, Behavioral, and Cognitive needs.
• Coordinate care with psych providers and ancillary services as needed.
• Educate residents and families about Advanced Directives and obtain required forms within 48 hours of admission.
• Greet residents and families, ensure daily visits and assessments, and document interactions in the Medical Record.
• Demonstrate proficiency with EMR and assist in staff training related to resident rights and Social Services.
