• Carry out plans to help residents reach full potential during their stay.
• Educate residents and families about post-acute options and discharge choices.
• Assist team with discharge planning activities.
• Complete and document social work assessments within 3 days of admission/readmission.
• Ensure required forms are completed and accurate on admission/annually.
• Schedule and notify IDT for 72-hour and quarterly/annual care conferences.
• Complete MDS sections and Care Assessments (CAA).
