The position requires a bachelor’s or master’s degree in social work or a related field, plus one year of supervised experience in healthcare, with licensure as a social worker in Michigan.

Qualifications

• Bachelor's or Master's degree in social work or a human services field as required.
• One year of supervised social work experience in a healthcare setting.
• Experience in long-term care, working with the senior population and dementia counseling.
• Must be licensed as a social worker in Michigan.
• Must lead by example and possess superior customer service skills and professionalism.
• Flexibility to adapt to ongoing change in a fast-paced, customer driven environment.

Responsibilities

• Participate in planning related to resident, family, and community interests; participate in discharge planning and development of social care plans.
• Promotes safe and effective transition for discharge and maintains communication with residents and responsible persons during stay.
• Provide information to residents/families on Medicare/Medicaid and other financial assistance programs; advise staff and community agencies to solve concerns through social services programs.
