Job summary

Valley View Health Care Center, INC seeks a Social Services Specialist to support residents and families as they adjust to placement, illness, and care plans. You will assist with assessments, rights information, and resource coordination within federal and state guidelines.

You will also help with appointments, transitions, and discharge planning, working closely with the Social Services Director to ensure high-quality, person-centered care. This role is onsite in Colorado.

Qualifications

• Geriatric experience preferred
• AA or Bachelor’s degree preferred

• Professional interaction with residents, families, staff, and external agencies

Responsibilities

• Interview and gather information for psychosocial assessments and care planning
• Assist with transition and adjustment to facility placement
• Ensure resident rights and preferences are respected and documented
• Coordinate appointments and transportation for residents
• Assist with financial, legal, mental health, and community resources
• Participate in resident and family council meetings
• Support discharge planning and follow-up services
