Job summary

Valley View Care Center is seeking a qualified Social Worker in California to identify residents' psychosocial, mental, and emotional needs and to assist in accessing services in line with NASW ethics and regulatory guidelines.

You will conduct social histories, contribute to MDS, develop interdisciplinary care plans, educate families, coordinate discharge planning, and support resident rights within a long-term care setting.

Qualifications

• Certification/licensure required in state of practice.
• Bachelor's Degree in Social Work or related Human Services field (Sociology, Special Education, Rehabilitation Counseling, Psychology) from an accredited school.
• One year of supervised social work experience in a healthcare setting with geriatric residents.
• Minimum of 2 years nursing experience in a Skilled Nursing Facility preferred.

Responsibilities

• Completes social history and psychosocial assessment for residents identifying social, emotional, and psychological needs.
• Maintains accurate documentation compliant with regulations and center policies including MDS, care plans, and progress notes.
• Supports resident self-determination and informs about therapeutic alternatives when needed.
• Participates in interdisciplinary plan of care addressing psychosocial needs and goals.
• Provides therapeutic interventions to aid transition and adjustment to long-term care.
• Supports residents and families with placement understanding and referrals to social service agencies.
• Coordinates discharge planning and referrals for home care services.
• Collaborates with ITnterdisciplinary team to protect resident rights and psychological well-being.
