Job summary

Desert View is a skilled nursing and rehabilitation community in Buhl, Idaho, seeking a Social Services professional for full‑time days. You will coordinate social work services, communicate with families, manage referrals, and support residents’ dignity through interdisciplinary planning.

Qualifications

• One year of supervised social work experience in a healthcare setting preferred.
• Experience with electronic medical records (EMR) required or preferred.
• Basic computer skills including Word and Excel.
• HS diploma or equivalent required.
• Bachelor's in Social Work or Human Services required.
• Master's level social work licensure preferred.
• CSW or LCSW helpful but not required.
• Exposure to body fluids and communicable diseases.
• Full range of motion; lifting objects up to 50–100 lbs.
• Adequate vision and hearing (with or without devices).

Responsibilities

• Arranges equipment, clothing, personal items, and counseling services for residents.
• Maintains communication with families on health updates and care planning.
• Coordinates referrals and access to outside services.
• Assists residents with financial and legal matters including Social Security and VA benefits.
• Promotes positive relationships and upholds resident dignity and individuality.
• Ensures confidentiality of social services and resident information.
• Participates in care plan conferences and contributes to planning.
• Assists in resolving grievances from residents or families.
• Completes social service assessments, discharge planning, and MDS documentation in EMR.
• Documents resident social service needs, interventions, and goals in care plans.
• Performs additional duties as assigned.

Education

Bachelor's degree in Social Work or Human Services

High school diploma or equivalent

Master's level social work licensure preferred

CSW or LCSW helpful but not required
