Job summary

CareRite Centers, LLC is seeking a dedicated Social Worker to support residents' adjustment and psychosocial functioning within the facility. The role includes discharge planning, documentation, and coordination with families and staff to ensure smooth transitions and adherence to regulations.

You will maintain social service records, assist with advanced directives, and collaborate with interdisciplinary teams to meet residents' evolving needs.

Qualifications

• Two years experience in the field of social work in a long term care environment is required.

Responsibilities

• Support facility philosophy of care and professional interactions with residents, families, and staff.
• Greet new residents, orient them to the building and staff.
• Maintain and update social service documentation: initial admission notes, social history, social assessment, discharge planning as appropriate.
• Complete the Social Service Section of the MDS form on admission, quarterly, annually, and after significant changes.
• Coordinate room changes and communicate changes to involved departments with documentation in resident records.
• Provide information about Advanced Directives and assist in completing documents; maintain DNR forms and related lists.
• Coordinate discharge planning for appropriate residents, including arranging care giving or transportation as needed.
• Perform other related duties as directed by the Administrator.

Education

B.A. in Psychology or Sociology

B.A. or M.A. in Social Work
