Job summary

Elm Terrace Gardens in Lansdale, PA is seeking a social services professional to plan, develop, organize, implement, evaluate, and direct the social services program for residents in our Skilled Nursing Facility. Qualifications

• Must be at least eighteen (18) years of age and have completed high school or equivalent.
• Must possess a bachelor’s or graduate degree in an accredited social work or human services program approved by state/federal regulation.
• Must have a minimum of two (2) years of supervised social work experience in a health care setting, preferably Skilled Nursing.
• Must possess the ability to read, write, and follow oral and written directions; able to communicate to residents and staff.
• Must have an agreeable and helpful attitude toward senior adults.

Responsibilities

• Plan, develop, organize, implement, evaluate, and direct the social services program addressing residents' mental, emotional, and social needs.
• Consult with administration, medical, nursing services, and other departments; assist staff in understanding residents' needs.
• Coordinate social services programs with residents, families, staff, and community agencies through interviews and involvement.
• Provide information to residents and families about available support agencies and financial assistance programs.
• Promote Elm Terrace Gardens' mission and programs to residents, staff, visitors, and other stakeholders.
• Coordinate admissions with the Nursing Home Administrator and assist with admissions to ancillary units; contact hospitals/referral resources for new admissions.
• Complete admission paperwork and assist with Medical Assistance processes and determinations.
• Observe changes in resident behavior and report promptly to nursing staff.
