Abington Manor in Lackawanna County, PA is seeking a full-time Social Service Coordinator with a Bachelor's degree in Social Work or related field and experience with the elderly population. The role supports residents in a long-term care setting, focusing on psychosocial assessment and access to needed services.

You will collaborate with a multidisciplinary team, advocate for residents' rights, and assist with admissions, care planning, and clinical interventions.

Qualifications

• Bachelor's Degree in Social Work or related field required.
• Minimum three years supervised social work experience in health care setting preferred.
• Prior work with elders in a community or long-term care setting preferred.
• Must have communication, assessment and social work methods/techniques.
• Ability to work with diverse disciplines and understand state/federal regulations.
• Must complete post-offer, pre-employment background check and drug screen.

Responsibilities

• Assist the Director of Social Services.
• Identify residents' psychosocial, mental and emotional needs and facilitate access to services.
• Exercise professional judgment to maximize well-being and quality of life.
• Perform Administrative duties, advocacy and residents' rights, clinical assessment and care planning.
• Establish referrals and assist with admissions per policies and supervisor guidance.
• Conduct tours and initiate admissions data collection.
• Undertake other duties as assigned.
