Qualifications

• Must be at least eighteen (18) years of age and have completed high school or equivalent.
• Must possess a baccalaureate or graduate degree in an accredited social work or human services program, approved by state/federal regulation.
• Minimum one (2) year supervised social work experience in a health care setting; SNF preferred.
• Must be able to speak to and be understood by residents and staff, and read, write, and follow directions; maintain a helpful attitude toward senior adults.
• Must possess an agreeable and helpful attitude toward senior adults.

Responsibilities

• Plan, develop, organize, implement, evaluate, and direct the social services program for Elm Terrace Gardens, addressing residents' mental, emotional, and social needs.
• Consult with administration, medical, nursing services, and other departments to support residents' social and emotional needs.
• Coordinate social services activities with residents, families, staff, and community agencies.
• Provide information about available support agencies and financial assistance programs.
• Promote Elm Terrace Gardens' mission and policies to residents, staff, visitors, and other stakeholders.
• Coordinate admission with the Nursing Home Administrator and assist with admissions to the Personal Care Unit; contact hospitals and referral resources.
• Complete all admission paperwork and assist with Medical Assistance processes and Options determinations.
