Job summary

Elm Terrace Gardens in Lansdale, PA is seeking an experienced Social Services Program Coordinator to guide a medically related program for residents and families.

The role collaborates with the Interdisciplinary Care Team to address social service needs, conduct assessments, document quarterly notes, and coordinate discharge planning.

A bachelor’s or graduate degree in social work or human services plus 2+ years in healthcare social work is required; full benefits are provided.

Qualifications

• Must possess a baccalaureate or graduate degree in an accredited social work or human services program
• Minimum of two (2+) years of supervised social work experience in a health care setting
• Ability to speak to and be understood by residents and staff; read, write, and follow oral and written directions
• Demonstrated ability to work with seniors and provide compassionate care
• Experience developing and managing Social Service Programs in a CCRC or related setting

Responsibilities

• Participate as a member of the Interdisciplinary Care Team to address the Social Service sections of the MDS and care plan
• Participate as a member of the Interdisciplinary Care Team to address resident and family concerns
• Participate in Quality Assurance program and other committees to evaluate residents’ needs
• Attend care conferences, resident council meetings and related sessions
• Coordinate care conference schedules with residents and families
• Conduct social services assessments of new and current residents and discharge planning
• Assist with addressing behavioral problems through 1:1 visits and document visits thoroughly
• Maintain social service records and make appropriate entries in medical records
• Ensure forms and reports are completed with informative descriptions of services
• Observe changes in resident behavior and document them in the social services section of the medical chart
• Develop and maintain rapport with facility services, residents, staff, families, and support agencies

We are seeking an experienced Social Services ProgramCoordinator to join our Team!

Purpose of the Social Services Program Coordinator:

To plan, develop, organize, and direct a medically relatedSocial Services Program for Elm Terrace Gardens residents per the following:

• (1) According to applicable current federal, state, and localregulations and standards and as may be directed by the NursingHome Administrator
• (2) According to the mission of Elm Terrace Gardens to promoteand maintain the highest practicable physical, mental, andpsychosocial well-being of each resident. and to prevent theoccurrence or progression of personal and social problems.

Resident Assessment & Review Functions - Duties:

• Participate as a member of the Interdisciplinary Care Team toaddress the Social Service sections of the MDS and care plan thathas been developed.
• Participate as a member of the Interdisciplinary Care Team toaddress resident and family concerns and to get information towrite an adequate quarterly note.
• Participate as a member of the Quality Assurance program, andas a member of other committees or groups as may be directed forthe purpose of identifying and evaluating the physical, mental,and psychosocial needs of resident
• Attend Quality Assurance Committee meetings, care conferences,resident council meetings and such other sessions as directed by theNursing Home Administrator.
• Coordinate care conference schedules with residents andresident families or responsible parties.
• Conduct social services assessments of new residents,reassessments of current residents, and discharge planning inaccordance with documentation and time-frame requirements,including the Minimum Data Set, individual care plans, anddischarge forms.
• Assist with addressing behavioral problems through 1:1 visitsand document these visits thoroughly.
• Maintain social service records and make appropriate andspecific entries in each resident's medical records.
• Ensure that other necessary forms and reports are properlycompleted, and that entries and reports contain informativedescriptions of services provided.
• Complete all required documentation in a timely fashionaccording to federal and state guidelines and facility policy.
• Observe and promptly report to the nursing staff changes inresident behavior or condition, especially symptoms or appearancesof depression, anxiety, withdrawal, or aggression; immediatelydocument these changes in the Social Services section of themedical chart.
• Develop and maintain a rapport with Facility services,residents, staff, and consultants, families, and support agenciesto ensure an effective program of care and quality of life for residents.

Experience and Skills:

Qualifications required:

• Must possess a baccalaureate or graduate degree in an accreditedsocial work or human services program which is also approvedaccording to current state and federal regulation; and
• Must have a minimum of two (2+) years of supervised social workexperience in a health care setting working directly with individuals
• Must possess the ability to speak to and be understood byresidents and staff, and demonstrate the ability to read, write, andfollow oral and written directions
• Must possess an agreeable and helpful attitude toward senior adults
• Must have prior experience with developing and managing SocialService Programs in a CCRC, or related setting.
