Job summary

Petaluma Skilled Nursing Facility in Petaluma, CA is seeking a licensed Physical Therapist to join our team. You will evaluate patients, develop individualized treatment plans, and guide recovery for geriatric and post-acute populations in a SNF setting.

Qualifications

Active California licensure as a Physical Therapist.
Current BLS/CPR certification.
Minimum of 1 year of PT experience, preferably in a SNF/geriatric setting.
Strong clinical reasoning and leadership skills.
Ability to work collaboratively in an interdisciplinary team.

Responsibilities

Evaluate, diagnose, and treat patients with physical impairments in a Skilled Nursing Facility setting.
Develop and implement individualized treatment plans based on patient needs and clinical findings.
Supervise and collaborate with Physical Therapist Assistants to ensure effective care delivery.
Monitor patient progress and modify treatment plans as needed.
Educate patients and caregivers on mobility, safety, and home exercise programs.
Maintain accurate and timely documentation in compliance with facility and regulatory standards.
