Job summary

Manchester Center for Rehabilitation and Healing is seeking a Speech Language Pathologist (SLP) for full-time and PRN positions in our Skilled Nursing facility. The SLP will evaluate and treat geriatric patients, develop individualized treatment plans, and collaborate with the rehab team to enhance residents’ quality of life.

The role requires a TN license, experience in documentation, and a commitment to patient-centered care.

Qualifications

• Must have graduated from an accredited Speech-Language Pathology program.
• Must hold a valid TN state license and registration.
• Experience with long-term care or geriatric populations preferred.
• Strong documentation and patient‑centered care skills.
• Ability to collaborate with multidisciplinary team and manage time well.

Responsibilities

• Assess referred patients and develop rehab treatment plans.
• Provide direct patient care with goals tailored to capacity.
• Document progress notes, discharge summaries, and family education.
• Ensure compliance with HIPAA and confidentiality.
