Job summary

Manchester Center for Rehabilitation and Healing is seeking a licensed Speech Language Pathologist (SLP) to join a skilled nursing facility team. Qualifications

• Graduated from an accredited Speech-Language Pathology program.
• Must hold a valid TN license and state registration.
• Experience in long-term care or geriatric settings preferred.
• Strong documentation writing skills.
• Experience with evaluations, progress notes, recertification and discharge planning.

Responsibilities

• Assess referred patients requiring rehabilitation and provide direct patient care.
• Develop and administer individualized treatment plans with goals per patient capacity.
• Document patient progress and educate patients and families in therapy methods.
