Job summary

Manchester Center for Rehabilitation and Healing seeks a dedicated Speech Language Pathologist (SLP) to join our skilled nursing team. The SLP will assess referred geriatric patients, develop and implement treatment plans, and document progress while educating patients and families.

Candidates must have a Master’s in Speech-Language Pathology, TN licensure, and be able to work independently within a multidisciplinary team to deliver patient-centered care.

Qualifications

Graduated from an accredited Speech-Language Pathology program.
Must hold a valid TN license and state registration.
Experience in long-term care or geriatric care preferred.
Strong documentation and record-keeping skills.
Ability to conduct evaluations and progress notes independently.
Collaborates with disciplines for patient-centered care.
Punctual, professional, and respectful of confidentiality.

Responsibilities

Assess referred patients requiring rehabilitation and provide direct care.
Develop and administer treatment programs with specific goals.
Document patient conditions and progress; educate patients and families.
Coordinate with physicians and the care team to optimize outcomes.
