Job summary

Manchester Center for Rehabilitation and Healing is seeking a licensed Speech Language Pathologist (SLP) to join our skilled nursing facility team. The SLP will independently assess and treat geriatric residents, develop treatment plans, document progress, and educate families about therapy methods.

The ideal candidate holds TN licensure, has experience in long-term care, and demonstrates strong documentation and patient-centered care, professionalism, and teamwork with other disciplines.

Qualifications

• Must have graduated from an accredited Speech Therapist college or university.
• Must have a valid professional license and registration for state of TN
• Experience with long-term care and/or geriatric care is preferred
• Well versed with documentation writing
• Evaluations, weekly progress notes, recertification and discharge documentations.
• Collaborates with all disciplines to provide best patient centered treatment and care.
• Exceptional patient service skills and ability to demonstrate clinical excellence
• Report to work on time, adhere to scheduled hours and demonstrate professionalism at all times.
• Perform all other duties assigned by Director of Rehabilitation
• Compliance with confidentiality, HIPPA and Federal Residents Rights.

Responsibilities

• The Speech Language Pathologist is responsible for assessment of referred patients requiring rehabilitation and direct patient care.
• Establish and administer a treatment program with goals determined by patient capacity and physician direction.
• Document and educate patients and families in appropriate therapy methods.
