Ascension is hiring a Speech-Language Pathologist for the St. Vincent PACE program in Indianapolis, IN. This on-site role supports patients with communications, swallowing, and cognitive needs as part of an interdisciplinary team.

You will conduct FEES-based swallow assessments, develop individualized treatment plans, perform home and facility visits, and educate caregivers while advocating for older adults with advanced dementia.

Qualifikationen

• BLS Provider within 30 days of hire. American Heart Association or American Red Cross accepted.
• ASHA Speech-Language Pathologist credential; provisional license accepted until issued; must obtain within 12 months of hire date.

Aufgaben

• Deliver clinic and bedside speech, language, swallowing, and cognitive therapies with the interdisciplinary care team.
• Conduct swallowing evaluations (including FEES) and design tailored treatment plans to maximize safety and independence.
• Perform home and care facility visits, participate in daily IDT meetings, and educate caregivers on care plans.
• Advocate compassionately for older adult participants, specializing in advanced dementia care.
