Job summary

Oasis Therapy Services in Minnesota seeks a Speech-Language Pathologist to provide one-on-one rehabilitation in independent living, assisted living, and memory care sites in Litchfield. This PRN role focuses on speech, language, cognitive-communication and swallowing therapy to support residents' safety and independence.

You will conduct evaluations, develop treatment plans, educate residents and families, and collaborate with a multidisciplinary team.

Qualifications

• Active and current Minnesota MDH SLP license enabling independent practice.
• Master's or doctoral degree in Speech-Language Pathology.
• CCC-SLP from ASHA preferred.
• Current CPR certification.
• Strong clinical assessment, documentation, and communication skills.
• Ability to work independently and manage a PRN caseload.
• Experience in geriatric rehabilitation, dementia, and dysphagia preferred.

Responsibilities

• Conduct comprehensive SLP evaluations for speech, language, voice, cognition, and swallowing.
• Develop and implement individualized treatment plans to address communication and swallowing needs.
• Provide one-on-one interventions including cognitive-communication therapy and dysphagia management.
• Evaluate swallowing difficulties and recommend safety strategies and diets.
• Support residents with dementia through cognitive-communication interventions and daily activities.
• Monitor progress, adjust plans, and complete reassessments and discharge planning.
• Educate residents and families on strategies and home exercise programs.
• Collaborate with physical, occupational therapy, nursing, and physicians to coordinate care.
• Document evaluations, progress notes, and discharge summaries in Net Health.
