Job summary

Choice in Aging is seeking a Registered Nurse Care Manager (RNCM) - Casual to support frail, low-income elders enrolled in the MSSP Medi-Cal Waiver. The RNCM evaluates clients holistically and coordinates health and social services to promote safe community living.

Under supervision of the MSSP Site Director/Supervising Care Manager, the role includes in-home assessments, care planning, and collaboration with the CiA team to meet participants' medical and social needs with dignity.

Qualifications

• Bachelor’s Degree in Nursing (public health, health education, health administration, gerontology, or equivalent).
• 1–2 years of clinical nursing experience, preferably with frail older adults or 1 year of case management.
• Valid CA nursing license; CA public health certificate a plus.
• English communication skills in writing and speaking.
• Valid CA driver’s license, clean DMV record, automobile insurance.
• Ability to manage care for vulnerable, crisis-prone participants.
• Strong computer literacy and Excel proficiency.
• Current first aid/CPR certification.
• Pass DOJ fingerprint clearance.

Responsibilities

• Screen MSSP applicants to determine level of care.
• Conduct initial health assessment in home or by phone.
• Reassess medical portion of yearly review in person or by phone.
• Complete MSSP contracts (Application, Release of Information).
• Identify medical-related needs risking safety or placement.
