Job summary

Othello Post Acute seeks a rehabilitation professional to oversee and deliver patient care in a skilled nursing setting. You will evaluate patients, plan and provide therapies, educate staff and families, and document progress according to guidelines.

This role emphasizes safety, interdisciplinary coordination and ongoing quality improvement. You will supervise assistants, stay compliant with state licensure, and contribute to program development while ensuring timely billing and reporting

Qualifications

• Must maintain state licensure, certification and credentialing.
• Ability to supervise assistants and rehab aides.
• Willingness to participate in continuing education.

Responsibilities

• Makes evaluations initiated per MD orders and payer guidelines.
• Develops treatment plans and provides treatment for best functional outcomes.
• Educates staff and third party payers on benefits and outcomes.
• Completes documentation including evaluations, daily progress notes, and monthly reports.
• Screens individuals who may benefit from therapeutic intervention.
• Ensures safety of self, patients and others at all times.
• Participates in program development and new techniques to enhance utilization.
• Maintains licensure, certification and credentialing.
