Job summary

Habitat Health in Compton, CA, seeks a Functional Wellness Supervisor to lead the rehab team (PT/OT, aides, home care coordinators) at the PACE center. You will ensure high-quality, participant-centered therapy, oversee programs, and maintain compliance with CMS and licensure standards.

You will supervise daily operations, coach staff, analyze KPIs, and drive continuous improvement while collaborating with center leadership and the interdisciplinary team to promote independence and safety for

Qualifications

Unrestricted OT or PT license in the state of practice; bachelor’s degree from accredited program.
Two to four years of experience with at least one leading people and coordinating workflows.
Experience in geriatric rehabilitation, functional assessments, and evidence-based interventions.
Knowledge of PACE regulations, CMS requirements, and state licensure standards.

Responsibilities

Lead rehabilitation and functional wellness services at the Center to support independence, safety, and quality of life.
Oversee Occupational Therapy, Physical Therapy, and integrated wellness programming.
Align operations with PACE/CMS requirements and licensure standards.
Collaborate with interdisciplinary team to ensure participant-centered care plans.
Manage staffing, scheduling, and workflows; coach performance and coverage.
Monitor KPIs and drive quality improvement with audits and data-driven actions.
Standardize functional outcome measures and create reporting dashboards.
