Beaver Dam Health Care Center in Beaver Dam, WI, is seeking a dedicated Physical Therapist Assistant to join our care team. Under the supervision of a physical therapist, you will treat and document care for residents to promote independence and quality of life.

Responsibilities include following physicians’ orders, providing therapies to maintain or improve function, maintaining resident records, observing safety and infection control, and collaborating with the interdisciplinary team.

Qualifications

• Excellent verbal and written communication and organizational skills.
• Ability to read, analyze, and interpret common medical information.
• Associates degree from an accredited Physical Therapy Assistant program.
• Active Physical Therapy Assistant license.

Responsibilities

• Follow physicians’ orders for resident care under supervision of the PT.
• Provide treatment to help residents maintain or attain the highest level of function within their abilities.
• Maintain timely and accurate resident reports per regulatory requirements and payers.
• Observe infection control procedures and safety policies.
• Collaborate with residents, families, facility staff, physicians and consultants.
• Adhere to facility policies and procedures and Residents' Rights policies.
