Job summary

You’ll collect initial evaluation data, help prioritize care, and adjust interventions to achieve optimal outcomes, while training caregivers for discharge and coordinating with the health care team for smooth transitions.

Qualifications

• Associate degree from an accredited PTA program.
• Current State PTA license.
• CPR certification or willingness to obtain within six months.

Responsibilities

• Assist the Physical Therapist in delivering goal-directed treatments.
• Coordinate programs to restore function and maximize patient outcomes.
• Assess treatment effectiveness and adjust interventions as needed.
• Document progress and communicate discharge needs to the care team.
• Provide caregiver training for post-discharge care.
• Assist with orientation of staff and students as applicable.
