Qualifications

• Doctorate or Master’s degree in Physical Therapy.
• Current Illinois PT license and CPR certification.
• Experience working with adult and geriatric populations preferred.

Responsibilities

• Evaluate, plan, and provide evidence-based physical therapy services.
• Develop individualized treatment plans and document services timely.
• Provide gait training, balance retraining, strengthening, modalities, and functional mobility.
• Monitor equipment and ensure safe practice environments.
• Contribute to restorative and wellness programming.
• Supervise Physical Therapy Assistants per Illinois licensure rules.
