Qualifications

• Licensed Physical Therapist (PT) or license eligible in the state of practice.
• Interest or experience in skilled nursing, long-term care or geriatric rehabilitation.
• Passion for patient-centered care and improving quality of life for older adults.

Responsibilities

• Evaluate patients and provide personalized physical therapy treatments.
• Develop and implement patient-centered treatment plans.
• Deliver hands-on therapy interventions to support recovery.
• Help patients restore functional mobility and perform daily activities.
• Partner with interdisciplinary teams to drive meaningful patient outcomes.
• Track patient progress and adapt treatment plans based on needs.
