Job summary

Legacy Healthcare in Louisville area seeks a licensed Physical Therapist (PT) to join a PACE program. The role covers assessments, individualized therapy, and collaboration within an interdisciplinary team to improve mobility and safety for older adults.

The PT will conduct home and center visits, educate caregivers, and document progress according to program standards. A driver’s license is required.

Qualifications

• Licensed Physical Therapist (PT) in Indiana or eligible for licensure.
• Minimum 1 year of geriatrics, home health, SNF, outpatient, or PACE experience.
• Excellent interpersonal and communication skills; thrives in an interdisciplinary care model.
• Proficient in EMR systems and documentation best practices.
• Valid driver’s license and reliable transportation for home visits.

Responsibilities

• Evaluate participants' physical function, mobility, gait, balance, and rehab needs in home and center settings.
• Develop, implement, and modify individualized PT care plans with the interdisciplinary team.
• Provide therapies to improve mobility, reduce falls, and increase independence.
• Instruct participants, caregivers, and family on exercises, mobility, and assistive devices.
• Document assessments, treatments, and progress per PACE standards and regulations.
• Participate in care planning meetings and coordinate with RNs, PCPs, OTs, social workers, and others.
