Qualifications

• Must have medical clearance for communicable diseases and up-to-date immunizations.
• Must have a valid driver’s license and transportation to assist participants in their homes or other settings.

Responsibilities

• Perform physical therapy treatments and related duties as delegated by the physical therapist.
• Communicate participant progress to the Registered Physical Therapist.
• Maintain documentation according to state policies and PACE regulations.
• Maintain records in the EMR per policy and procedures.
• Perform planned data collection and other duties as required.
