Storybrook Care & Rehabilitation is seeking a compassionate Physical Therapist to join our in-house therapy team at a midsize facility in Northern Colorado. The role offers flexible scheduling with full-time, part-time or PRN options and pays $40-60 per hour plus a sign-on bonus.

You will develop individualized treatment plans, supervise assistants, coordinate with the rehabilitation team, and provide discharge summaries and family counseling as part of a patient-centered geriatric program.

Qualificações

• Active State Licensure
• Flexible schedule: full-time, part-time or PRN

Responsabilidades

• Develop individualized treatment plans and implement therapy treatments.
• Coach patients in exercises and therapeutic techniques to improve mobility.
• Maintain accurate patient records and discharge summaries.
• Supervise assistants and collaborate with the rehabilitation team.
• Communicate progress with physicians, families, and care team.
