Job summary

Tender Touch Rehab Services seeks a compassionate Physical Therapist Assistant to deliver high-quality rehab services to geriatric and sub-acute residents in skilled nursing facilities.

You will work under the direction of a Physical Therapist to implement individualized treatment plans, including therapeutic exercise, gait and balance training, and functional mobility activities to enhance safety and independence.

Responsibilities

• Provide rehab services to geriatric and sub-acute residents in skilled nursing facilities.
• Under supervision of a Physical Therapist, implement individualized treatment plans including therapeutic exercise, gait and balance training.
• Document progress and educate patients and families on treatment goals and outcomes.
• Collaborate with an interdisciplinary team to support patient safety and functional independence.
