Tender Touch Rehab Services in New Jersey seeks a compassionate Certified Physical Therapist Assistant to join our on-site Healthcare & Medical Services team. You will implement PT treatment plans for seniors in skilled nursing and long-term care, focusing on mobility, function, safety, and quality of life. You will document EMR, educate patients and caregivers, and work with an interdisciplinary team in a patient-centered culture. Support for mentorship and growth is offered.

Qualifications

• Requires PTA certification and state licensure.
• Experience in geriatric or long-term care settings preferred.
• Ability to work collaboratively in an interdisciplinary team.

Responsibilities

• Perform PT-related therapeutic exercises and functional activities.
• Conduct gait and balance training to improve mobility and safety.
• Document therapy sessions accurately in EMR and communicate progress.
• Educate patients and caregivers to support ongoing care.
• Collaborate with nursing and therapy teams to optimize care.
