Job summary

Tender Touch Rehab Services is seeking a compassionate Certified Physical Therapist Assistant to join our Healthcare & Medical Services team on-site in New Jersey. You will implement PT treatment plans focusing on improving mobility, function, safety, and quality of life for seniors in skilled nursing and long-term care settings.

You will collaborate closely with an interdisciplinary team, document accurately in the EMR, and provide patient and caregiver education.

Qualifications

• Certified Physical Therapist Assistant with current licensure.
• Experience in geriatric therapy is preferred.

Responsibilities

• Provide individualized therapy to seniors in skilled nursing and long-term care settings.
• Perform therapeutic exercises and gait/balance training to improve mobility and safety.
• Develop functional activities and educate patients and caregivers.
• Accurate EMR documentation and collaboration with an interdisciplinary team.
