Tender Touch Rehab Services is seeking a compassionate Certified Physical Therapist Assistant to join our Healthcare & Medical Services team, providing individualized therapy to seniors in skilled nursing and long-term care settings. This on-site role implements PT treatment plans focused on improving mobility, function, safety, and quality of life.

You will collaborate with an interdisciplinary team in a patient-centered culture that values dignity and clinical excellence.

Qualifications

• Licensed PTA in Pennsylvania (CPTA).
• Experience in skilled nursing settings preferred.
• Strong documentation in EMR and patient education.

Responsibilities

• Implement PT treatment plans to improve mobility, function and safety.
• Perform therapeutic exercises and gait/balance training.
• Document progress accurately in EMR.
• Educate patients and caregivers to support ongoing care.
