Job summary

Park Place Healthcare & Rehab is seeking a dedicated physical therapy support professional to assist the rehabilitation team in providing resident care. The role involves following the Physical Therapist's direction, monitoring progress, documenting treatments, and participating in care conferences.

You will contribute to safety, confidentiality, and infection control while supporting families and staff in maintenance programs as directed by the Physical Therapist.

Qualifications

• Associate's degree required.
• Current license in state of practice.
• Certification, if applicable, and demonstrated competency in identified areas of specialization.
• Organized and detailed in work performance.
• Good communication skills with excellent self-discipline and patience.
• Genuine caring for and interest in elderly and disabled people in a nursing facility.
• Comply with the Residents' Rights and Facility Policies and Procedures.

Responsibilities

• Treats residents as directed by the Physical Therapist and contributes to the PT's effectiveness.
• Monitors patient progress and maintains records and reports with confidentiality.
• Documents patient care services by charting in records.
• Participates in resident care conferences and weekly rehabilitation meetings.
• Communicates with supervisor and interdisciplinary team regarding progress, problems, and plans.
• Participates in facility in-service training programs.
• Records daily treatments and labor according to procedure.
• Instructs families and staff in maintenance programs as directed by the PT.
• Practices safety, infection control, and environmental safeguards.
• Be familiar with procedures and know how to use information.
