Job summary

Vitae Health Systems seeks a qualified Psychotherapist to deliver mental health services to residents in a skilled nursing facility. You will assess, diagnose, and treat emotional and behavioral conditions, supporting residents’ well‑being and adjustment to medical life changes.

Responsibilities include psychosocial assessments, treatment planning, various therapy formats, and collaboration with an interdisciplinary team, with strict HIPAA compliance and careful documentation.

Qualifications

• Master’s degree in Psychology, Counseling, Social Work, or related field and active licensure in state.
• Strong clinical assessment, diagnostic, and treatment planning skills.
• Knowledge of SNF mental health needs and geriatric care.

Responsibilities

• Conduct psychosocial assessments for residents at admission and as needed.
• Develop, implement, and monitor individualized treatment plans.
• Provide individual, group, and/or family therapy sessions.
• Collaborate with the clinical team, attending physician, nursing staff, social workers, and rehabilitation teams.
• Maintain HIPAA-compliant documentation and participate in care plan meetings.
• Provide crisis intervention and staff education on mental health strategies.
