Job summary

Vitae Healthis is seeking a qualified Psychotherapist to provide mental health services to residents in a skilled nursing facility. The Psychotherapist will assess, diagnose, and treat emotional and behavioral conditions, supporting residents’ well-being and adjustment to medical changes.

Qualifications

• Master’s degree in Psychology, Counseling, Social Work, or related field required.
• Active and unrestricted license in the respective state (e.g., LCSW, LPC, LMFT, or Licensed Psychologist).
• At least 2 years of clinical experience preferred, ideally in geriatric, long-term care, or medical settings.
• Strong clinical assessment, diagnostic, and treatment planning skills.
• Knowledge of mental health conditions common in SNF populations (depression, anxiety, dementia, adjustment disorders).
• Proficiency in evidence-based therapeutic approaches (CBT, trauma-informed care, grief counseling).
• Ability to work collaboratively within an interdisciplinary team.
• Excellent communication and interpersonal skills with residents, families, and staff.
• Strong organizational, time management, and documentation skills.
• Familiarity with electronic health records (EHR) systems preferred.

Responsibilities

• Conduct comprehensive psychosocial assessments for residents on admission and as needed.
• Develop, implement, and monitor individualized treatment plans.
• Provide individual, group, and/or family therapy sessions.
• Address depression, anxiety, grief, cognitive decline, and adjustment to illness or disability.
• Support residents coping with chronic illness, pain, loss of independence, and end-of-life concerns.
• Collaborate with the Vitae clinical team, attending physician, nursing staff, social workers, and rehabilitation teams.
• Participate in care plan meetings and interdisciplinary discussions.
• Maintain accurate clinical documentation in compliance with regulations.
• Provide crisis intervention and behavioral management strategies.
• Educate staff and families on mental health and effective communication.
• Ensure HIPAA compliance and ethical practice.
