Job summary

Eventus WholeHealth seeks licensed mental health clinicians to join our multidisciplinary team serving adults in skilled nursing and assisted living facilities. You will provide recovery-oriented rehabilitation services in group or individual settings and collaborate with physicians, nurse practitioners, physician assistants, and support staff to improve residents' quality of life.

Qualifications

• Psychologists: Doctoral degree and doctoral-level license required.
• Therapists: Master’s degree in social work and current state license to practice required.

Responsibilities

• Assess each resident with consent to determine needs and develop a treatment plan.
• Consult with primary care physician, family, facility staff and support staff as needed.
• Document treatment plans, psychotherapy sessions, and updates within 48 hours for medical records.
• Meet with facility staff to review care needs and communicate findings.
• Communicate treatment plans and pertinent caregiver information to staff.
• Adhere to Eventus WholeHealth standards, policies and regulatory rules (HIPAA compliant).
• Maintain patient confidentiality and professional conduct at all times.
• Consult with clinical supervisor on unfamiliar requests regarding resident care.
