Job summary

Qualifications
- Requires an Unrestricted Illinois license.
- Board Certified as an APRN or PA.
- Minimum 5 years of relevant experience; primary care/family medicine experience preferred.
- BLS certification required.
- NCCPA certification and DEA license preferred.
Responsibilities
- Deliver comprehensive outpatient primary care services in an outpatient geriatric setting.
- Support patients with geriatric needs and coordinate referrals and specialty care.
- Collaborate with a dedicated team and utilize EPIC EMR with DAX Copilot support.
- Maintain work-life balance with limited call responsibilities and no holidays or weekends.
