Community Physicians in Illinois seeks a Board-certified NP or PA to join our outpatient geriatric program. You will co-manage a defined panel with a lead physician, collaborate with a dedicated team, perform preventative screenings, review medications, and order labs in a fast-paced clinic.

Ideal candidates will have 3+ years as an NP, interest in geriatrics, and strong communication with patients and families.

Qualifications

• Board-certified NP or PA required.
• 3+ years of Nurse Practitioner experience preferred.
• Interest in geriatrics.
• Comfortable working in multiple care settings.
• Strong communication with patients and families.
• Detail-oriented and collaborative clinician.

Responsibilities

• Co-manage a defined panel of geriatric patients with a lead physician.
• Collaborate with a team of healthcare professionals in an outpatient clinic.
• Perform preventative screenings and discuss long-term goals of care.
• Review and update patient medication lists and order labs/diagnostics.
• Maintain patient records accurately and efficiently.
• Maintain independent clinical decision-making in a fast-paced environment.
• Provide care across the continuum, including post-acute settings like AL/IL and SNFs.
