Job summary

Oak Street Health in Durham, NC is seeking a Primary Care Physician to deliver equitable, value-based care to local Medicare populations within a neighborhood center. You will work with a dedicated care team to focus on outcomes and patient experience, rather than volume of services.

The role emphasizes collaboration with leadership, adherence to high-quality care standards, and participation in a comprehensive care program designed for older adults. A 40-hour work week is anticipated.

Qualifications

• MD or DO graduate.
• Internal Medicine or Family Medicine board certification (or eligible).
• Active state medical license (no probation).
• Active DEA license required.
• US work authorization required.

Responsibilities

• Assess and diagnose patients at local clinics.
• Oversee, direct and administer primary care.
• Prescribe medications and manage treatment plans.
• Maintain patient EMR data in canopy and Greenway.
• Collaborate with leadership to meet health quality goals.
• Work with practice managers to direct the center care team.
• Additional duties as assigned.
