Qualifications

• MD or DO graduates with a medical license and board eligibility/board certification in Internal Medicine or Family Medicine.
• Active, non-probationary state medical license required.
• Fellowship training in Geriatrics welcome but not required.
• Demonstrated ability to lead care teams and deliver patient-centered primary care.

Responsibilities

• Provide outpatient primary care to senior patients.
• Lead and collaborate with a multidisciplinary care team to deliver comprehensive care.
• Deliver high-quality, patient-centered care and address social and medical needs of patients.
